Tracy Bell

Tracy joined the team in December 2020 as the practice Social Prescribing Link Worker.  She receives referrals from the practice team to work with all patients who require support outside of medical needs.  The link worker role includes linking patients to community activities, supporting them to access benefits and housing aid and help to combat loneliness and isolation.  If you feel you need help in any of these or, another other aspects of your life, speak to your GP next time you see them , or you can ask for a referral through the reception team.
