Unable to work for less than 7 calendar days:
If you have been off work and ill for less than 7 days, your employer will provide you with a SC2 self-certificate form.
Unable to work for more than 7 calendar days:
If you need a sickness or a fit note after seven days please complete an e-Consult form via the home page or alternatively contact the surgery and speak to a receptionist. Please inform us if you would like to receive the note via text or post.
