If you suffer from a long term condition, your health will be monitored annually by the Practice. This normally consists of two parts.
You will initially receive a letter inviting you to see one of our health care assistants, this will be around your birthday month. During you visit, they will carry out some investigations, including blood tests, blood pressure, height and weight. You will also be asked some general questions about your health and lifestyle.
Once your investigation results are back, you will receive a follow up telephone appointment with one of our practice pharmacists who will make any indicated alterations to your medication regime.
