Long-Term Condition Reviews

Do I need a review if I have a long term condition?

Yes, if you suffer from a long term condition, your health will be monitored annually by the Practice. This normally consists of 2 parts.

How do I know when I am due a review?

You will initially receive a letter inviting you to see one of our Healthcare Assistants or our Practice Nurse around your birthday month.

What will happen during  the first part of my review?

During your visit, our Healthcare Assistant or Nurse will carry out some investigations. This was include: blood tests, blood pressure, height and weight. You will be asked some general questions about your health and lifestyle.

What happens next?

Once your investigation results are back, you will receive a follow up telephone appointment with one of our practice pharmacists who will make any indicated alterations to your medication regime.

Why are annual reviews important for long-term conditions?

The aim is to make sure your condition is being controlled as well as possible and to identify any areas where we can make changes to help you. This might include changes to the type or dosage of medication and check any equipment you might use, like inhalers, are working effectively.

What conditions class as long-term conditions?

Some examples of long-term conditions that need a review include but are not limited to;

  • Asthma / COPD
  • Diabetes
  • Hypertension
  • Learning Disabilities
  • Heart Failure
  • Stoke
