Can we also make you aware of the Little Orange Book, which provides expert help for parents and carers on how to manage common problems in babies and small children – ask your GP for a copy, or view it online here

Register at our practice
We are currently accepting new patient registrations at the Practice.
Registration forms can be collected from the surgery or downloaded and completed in advance. Please bring the completed forms plus appropriate proof of identity into the surgery where a receptionist will check your details.
If you wish to download the registration forms please choose the relevent pack from the following:
Patient Choice Scheme for Out of Area Registrations
We are not participating in the Patient Choice Scheme for Out of Area Registrations. This is for a number of reasons, the most important of which is that we have not yet been informed of what the urgent care arrangements will be for out of area patients who are unable to attend the surgery. We believe that this may impact on patient safety so we will continue to request that patients moving out of our practice area reregister with a new practice.
Online Patient Access
You can book appointments to see your doctor, place orders for repeat prescriptions and also view a summary of your medical records online.
To do this you need to register at reception please see our opening times. When you register , we’ll give you all the information you need to get going.
Click on the link below to see how we intend to improve this service.
Please click the link above (“Practice Leaflet”) to download our practice leaflet.
Patients are advised to refer to the guidance provided on the TravelHealthPro website:
Having read the above guidance we request that patients requesting NHS travel vaccinations download the travel risk assessment form, below, which is to be completed and returned to the surgery. Upon receipt of the completed travel risk assessment form our practice nurse will contact you to discuss your request further and, if required, offer you an appointment.
Please note that travel vaccinations do not take priority over medical appointments. We therefore advise that you submit your completed travel form in a timely manner (approximately 2 months before your date of travel).
We do not provide a private travel vaccination service therefore we are only able to offer vaccinations covered by the NHS programme. Private travel advice and vaccinations can be sought at many larger pharmacies.
Test results are available to patients via on-line access.
Test results take time to reach us. The practice will contact you should any treatments or follow up be required. If you do need to contact the surgery for test results please do so any weekday between 2pm and 5pm. Enquiries about tests ordered at the hospital should be directed to that hospital, not the practice.
Patient Participation Group
We are actively looking for patients to join our Patient Participation Group. The members meet between two and four times a year with staff members to discus practice matters and input from the members is greatly appreciated. If you are interested in joining the group please contact the surgery.
All calls to and from the practice are recorded for training and monitoring purposes. If you do not wish to have your calls recorded please notify the practice.
